Claude Code adds gateway picker, terminal OAuth

Claude Code v2.1.126 adds support for Anthropic-compatible gateways in the `/model` picker and lets users paste OAuth codes directly into the terminal when browser callbacks fail. The release also tightens Windows, MCP, remote-control, and session stability across a long list of fixes.
